High-value clinical care has been defined as “the best care for the patient, with the optimal result for the circumstances, delivered at the right price” (Institute of Medicine , USA). The consideration of an “optimal result” can be controversial if we don´t introduce the patient´s values or the expertise of the physician in charge…

The quality or type of clinical practice is directly influenced by the model of medical training, type of health services and cultural and geographical environment. But the first step to change practice is medical education and step by step we can see the introduction of concepts such as “teamwork”, communication skills and no less important “diagnostic reasoning”, as we show here in a recent paper published in JAMA, with the competencies developed by the Accreditation Council of Graduate Medical Education in USA.
