CLINICAL CASE: A 40-year-old man started a week before with high fever and low back pain, not related with the physical activity. No urinary infection or pneumonia was detected.

What are your diagnostic suggestions?

FINAL DIAGNOSIS: Acute Lymphoblastic Leukemia with bone marrow infiltration.

COMMENT: Acute leukemia is an agressive disease and usually the diagnosis is established inmediately based on the clinical picture and the high count of white cells  together with thrombocytopenia.
